Hazardous Waste Determination
For waste mixtures (wastes not in the original container), the Hazardous Waste Determination form below must be completed so EHS can caracterize the waste generated in your lab.

- Do I need to fill out this form?
- Yes, if you are generating mixed waste, and it is not in the original container, you do need to fill out this form.
- Why do I need to fill out this form?
- This is an easy way to help your lab and our department know if your waste is hazardous or not, and it'll help determine which waste label needs to go on the container.
- When do I need to fill out this form?
- This form should be filled out before waste is generated in the laboratory or if there is any new waste that will be generated.
- Do I need to label this waste the same as other hazardous waste generated in the lab?
- If the waste is hazardous, then you will need to use the Hazardous Waste Label template. If it's not hazardous, you can use the Non-Hazardous Waste Label template.
- Where can I find information on the Waste Characterization Pictograms?
- Both Princeton University and the Canadian Center for Occupational Health and Safety have comprehensive guides on what each pictogram covers, as well as examples of chemicals under each. Here are the pictograms below with general information:
